Medical Information Form

The
USA Equestrian Safety Committee is introducing a rider Medical
Information Form sponsored by Troxel Helmets, with the recommendation
that each rider/driver/handler complete a copy of the form and
place it in an outside coat, shirt or pants pocket whenever competing
at all recognized competitions. The committee advises that the
information exhibitors include in the form will assist medical
personnel in the event of a serious accident or injury. Please
inform your trainer and/or groom of the location of your completed
form and be careful if borrowing another exhibitors jacket.
Forms
are being mailed to competitions with the request that they be
made available to exhibitors in the show office. Forms are also
available as downloads on our website under Safety and under Competition
downloads and will be mailed to all members with their 2004 membership
cards.
USA
Equestrian Inc., as the National Equestrian Federation of the
U.S., is the regulatory body for the Olympic and World Championship
sports of dressage, driving, endurance, eventing, reining, show
jumping, and vaulting, as well as 19 other breeds and disciplines
of equestrian competition. As
the country's largest multi-breed organization, the Federation
has over 80,000 members and recognizes more than 2,800 competitions
nationwide each year. It governs all aspects of competition, including
educating and licensing all judges, stewards, and technical delegates
who officiate at these shows.
